Rheumatic MS – Anterior mitral leaflet (AML) shows diastolic doming or hockey-stick shape. And the posterior mitral leaflet (PML) has restricted motion or is totally immobile.
Changes associated with rheumatic mitral disease
- “Fish-mouth” appearance of the MV orifice,
- Commissural fusion, leaflet thickening
- Shortening and fusion of chordae tendinae
- These changes lead to the subsequent hockey-stick appearance of the anterior leaflet on echocardiography.
Why there is a diastolic doming or hockey-stick shape of AML?
- In case of Rheumatic Mitral stenosis there is reduced mobility of the valve leaflets.
- This doming is due to the reduced mobility of the valve tips compared to the base of the leaflets.
